How to Begin Care with Gilead Hospice
Anyone can refer a patient to hospice; patients can even refer themselves. Eligibility for hospice services must be certified by the patient’s attending physician and the hospice medical director.

Available Services
- Physician oversight for the medical direction of the patient’s care, provided by your physician or our medical director.
- Regular home care visits by nurses to monitor patient’s condition and to provide appropriate care and maintain patient comfort.
- Home Health aide attending to patient’s personal needs if needed.
- Chaplain services for emotional and spiritual support.
- Social work and counseling services.
- Bereavement counseling to help patients and their loved ones with grief and loss for up to 12 months following a death.
- Volunteer support to assist loved ones.
- Respite care (relief for family caregivers).
- Dietary counseling.
What is Hospice?
At the heart of hospice is the belief that each of us has the right to live till the end-of-life pain-free and with dignity, and that our families will receive the necessary support to allow us to do so, even in the face of advanced illness.
Hospice care involves a team-oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient’s needs and wishes. Support is extended to the patient’s loved ones.
Hospice care is provided by a specialized team made up of your attending physician or the hospice medical director, nurses, social workers, aides, spiritual counselors, and volunteers. The hospice team is on-call 24 hours a day.
